ABSTRACT

Medical ceitiikau generatur is registration and creating medical record certilicalc automatically for patient in the hospital. The medical certificate manager is the one with management responsibilities of an issuing certificate unit. They typically report to a service director. They have primary responsibilities for staffing, budgeting, and day-to-day operations of the unit, bed site teaching, complaint investigations &. conducting educational programs at unit base. I was motivated to embark on this research because of lire problem of accurate medical certificate generator, lack of immediate retrieval of information, the results ofmedical certificate generator in inconvenience using prevalent method and also waste a lot oftime using manual method, lack of immediate information storage, preparation of accurate and prompt reports becomes a difficult task due to lack of information collation. The system was achieved by developing a platform for medical certificate generator application system, build a database structure used to store data, design a platform that capture user registration, create a module that would create new' medical record, design a platform that can display medical certificate. This research was realized through the adoption ofprimary and secondary sources of data collection which includes interview, observation, questionnaire, the internet, journals and the adoption of waterfall model. The system is implemented by using HTML as front end, PUP as back end and MYSQL as the database.
